Amanda Anderson, LNHA
Administrator

Amanda is a seasoned Nursing Home Administrator with over 13 years of experience in long-term care; eight years of which she spent in facility leadership. Amanda began her career in Marketing and Admissions where she developed an expertise in resident intake and community outreach. These roles set the foundation for the passion she has in long-term care. She then pursued and obtained her Nursing Home Administrator License, transitioning into an Administrator role where she has successfully managed operations, compliance, and quality improvement initiatives.

Cassie Canfield, RN
Director of Nursing

With over 30 years in healthcare and 17 in leadership, Cassandra is a seasoned nursing professional committed to excellence. A summa cum laude graduate of The University of Akron, she blends clinical expertise with compassionate leadership to deliver personalized, high-quality care. Born at Medina General Hospital and a lifelong Medina County resident, she values her community deeply. Outside of work, Cassandra enjoys time with her family and exploring the beautiful parks that make Medina and its surrounding areas special.

Monica Bruggeman
Director of Admissions

Monica’s journey in long-term care began in 2013 after graduating from Bowling Green State University. For seven years, she served as Admissions Director at Riverview Pointe before joining the Champion Creek team. She loves connecting with residents, planning memorable events, and helping everyone feel at home. Her dedication earned her Manager of the Year and a spotlight on Fox 8’s Cleveland’s Own. Outside of work, Monica enjoys adventures with her husband and two children—especially visiting new zoos and waterparks during the summer.

Laura Clark
Director of Social Services

Laura has worked in long-term care since 2004 and joined Foundations in 2018. Along the way, she’s worn many hats—STNA, Medical Records, Admissions, Case Management, Business Office, and now Social Work. She thrives on solving new challenges and finds joy in helping others. Laura married her amazing husband in 2010, has four children, and is blessed with three beautiful grandchildren. In her spare time, she enjoys woodworking and power tools. Fun fact: Laura was the first baby born in Lorain County in 1978!

Jennifer Bloomfield
Director of Dining Services

Jenny Bloomfield joined Foundations Health Solutions in 2014 and has worked in food service since 2009. Starting as a cook, she rose through the ranks to become Director of Dining Services at Champion Creek. She was Employee of the Year in 2020 and won “Last Tray Standing” in 2022. Jenny takes pride in creating quality meals, planning events, and decorating for holidays. Outside of work, she enjoys photography, concerts, and trying new recipes. Jenny has a son named Aiden and has been happily married since September 2023.

Rita Innocenti
Business Office Manager

Rita began her long-term care journey in 1995 as a State Tested Nursing Assistant, moving into the Business Office in 1997 and later serving in regional roles for 15 years. She joined Foundations in 2019 as Business Office Manager at Brunswick Pointe. Rita takes pride in supporting residents, families, and staff while ensuring quality care behind the scenes. She values teamwork, transparency, and keeping residents at the heart of her work. Outside of work, Rita is married with two children and finds joy in celebrating life with her family.

Ashley Hagen, STNA
Director of Human Resources

Ashley began her career with Foundations in 2023 at Brunswick Pointe, bringing over 10 years of experience as an STNA in long-term care. Now stepping into the Human Resources role, she’s excited to support and celebrate the team. Born and raised in Medina, Ashley lives in Brunswick with her three children. She enjoys the outdoors, bike rides, swimming—and relaxing with a glass of wine. Ashley loves long-term care because of the time she gets to spend learning from the residents she serves.

Ron Smith
Maintenance Director

Ron joined Foundations in 2025 after owning his own business and came on board as our Maintenance Director. Ron has years of experience in building and construction. He has built houses, poured concrete, and is really good with his hands. He is always willing to help fix sometime or lend a hand. He loves spending time with his beautiful wife and 3 children fishing, riding four wheelers with his kids, or anything outside. He is excited to get to know the residents at Champion Creek!
