At Champion Creek, we prioritize personalized, compassionate care tailored to each resident’s unique needs. Our team works closely with patients and their families to create a comprehensive, individualized care plan that supports both physical and emotional well-being. Our goal is to foster confidence, independence, and a true sense of home in a welcoming and supportive environment.
What insurances do you accept?
Initially, we will be accepting Private Pay while we complete the State of Ohio certification process for full licensure. Once certified, we will be able to accept Medicare, Medicaid, and Medicaid Pending. Insurance contracts take time to apply and be approved as a participating provider, but we will continue to update our accepted insurance plans as they become available.
How does the admissions process work?
We strive to make the admissions process as smooth and stress-free as possible. Is there a waitlist for admission?

What does the daily rate include?
Our daily rate includes a private suite, 24/7 nursing care, state-certified aide coverage, meals, snacks, engaging activities, and access to a registered dietitian. Additional clinical modalities are also available. Medications are billed separately to your current insurance plan through our partner pharmacy.
What do I bring in for my loved one once they admit?
We welcome anything that will help our residents feel comfortable and at home. Personal items such as knickknacks, pictures, and memorabilia are encouraged to customize each room with the unique personality of our new family members. Champion Creek provides all linens, toiletries, and incontinence supplies. Please note that for safety reasons, certain items such as Keurigs, blow dryers, extension cords, curling irons, and live wreaths are not permitted.
Will my loved one still see their primary care physician?
Residents will transition to our onsite attending physician and nurse practitioner for general care. These practitioners work closely with our clinical team to ensure all residents' medical needs are met. However, residents are still able to see their specialists outside of the facility as needed.
When will Champion Creek officially open?
Our state-of-the-art facility is currently under construction, with a projected completion date of June 1, 2025. We look forward to welcoming residents and staff into our brand-new, thoughtfully designed center soon!
